HEALTH & CONSENT FORMS

  • Quaker Ridge has moved all health forms to electronic format. The health "form" will be a link to an online form and is required to be filled out there. More information has been sent directly to the Trip Leaders for each group/chapter attending.
  • EVERY PERSON that you bring to camp must fill out this form completely (including staff & volunteers). If students are missing anything, they will have to sit out from camp activities until the correct information is entered/signed in the electronic, online health record.
  • Please note that any activity vendor forms (i.e. Water Rafting, Rock Rappelling, etc) will be in paper form and not electronic for 2018.
  • Activity Waivers and Electronic Health Form information will be available in April 2018.

OFFSITE ACTIVITIES

On Day 5 of camp (Thursday, July 26th), everyone will need to leave camp for the day. The most popular option for that day is to go WHITE WATER RAFTING with Noah's Ark Rafting Adventures. On your Housing Form, you'll see a place to indicate how many people are planning to raft. Remember it is an additional per person charge of $75 per person. All camper/leaders participating MUST fill out the Noah's Ark Release Form which will be an online waiver system starting in 2018. We will be adding the rafting cost to your final camp bill.

Other things to do include:

Make sure you have plans for something to do for all of your students and leaders. You MUST plan to eat lunch OFFSITE and come back around 3pm.

ON A SEPARATE DAY = We will be going Rapelling or Rock Climbing. YOU NEED YOUR OWN TRANSPORTATION FOR THIS. However, there is no extra cost for the activity. We will contact you 2-3 weeks out with your group assignment so that you can make the necessary travel arrangements. - Release Forms will be available in April 2018.
